A portion of a data-entry window is not appearing on my monitor. How can I correct that?

You need to reset your screen resolution to a higher setting. The required minimum resolution when using your CSA software is 800 x 600 pixels. However, a resolution of 1024 x 768 pixels (or higher) is recommended.
