Note: After setting up a Third-Party Administrator account via the Michigan Web Account Manager (MiWAM) website at www.michigan.gov/uia, you can use the Power-of-Attorney upload file to associate clients with your third-party account so that you can bulk file and perform other functions for your Michigan clients. To upload the Power-of-Attorney Enrollment file, open the Michigan Payroll Tax Processing window and then choose File > e-Enrollment. For more details, see Enrolling clients for Michigan electronic filing.

Form UIA-1028

When you are ready to prepare the form, follow these steps.

  1. Choose File > Print when the Michigan Payroll Tax Processing window is active.
  2. Mark the Create SUTA file checkbox and then click OK.
  3. Choose File > SUTA File Utilities to do any of the following:
    • Combine Files
    • Preview Files
    • Verify
    • Remove Clients
    • Save to Submittable Format
    • Restore from Submittable Format
  4. Choose File > SUTA File Utilities > Save to Submittable Format, choose the file location, and then click OK.


    • When the file is created, it is automatically encrypted for security. Before submitting the file to the appropriate agency you must use the Save to Submittable Format command, at which point the files are decrypted.
    • File format: Quarterly Reporting/Wage Record Bulk file, and Bulk Payment file.
  5. Navigate to the Michigan Unemployment Insurance Agency website and follow the links to the MiWAM (Michigan Web Account Manager) to upload UIA-1028 files and Michigan Power of Attorney Client enrollment files (which are available to upload for any firm that is operating a Third Party Administrator account on the MiWAM site).
