How do I prepare Form 433-F with Form 9465?

Answer

In some scenarios, Form 433-F, Collection Information Statement must be filed as an attachment to Form 9465, Installment Agreement Request. If filing Form 433-F with Form 9465, prepare the Form 433-F as you would normally, and then enter X in the File Form 433-F with Form 9465 field on Screen 433-2 in the Payments folder. This will disable the Form 433-F filing instructions so that the form can be filed with Form 9465.
