Use the January - December Monthly Information dialog to enter Box 14, Box 15, Box 16, and Box 17 coverage code and premium amount information for individual months to be reported on Form 1095-C.

Choose Setup > Employees, click the Personal tab, click the Part II: Offer and Coverage button, and then click the Ellipsis ellipsis button button for the reporting year.

Fields & Buttons

January - December Monthly Information grid. Choose the Box 14 coverage code, enter the lowest cost monthly premium amount, choose the Safe Harbor code (if applicable), and enter the Box 17 business or employee zip code (if applicable) for each month.
