Use the January - December Monthly Information dialog to enter Box 14, Box 15, and Box 16 coverage code and premium amount information for individual months to be reported on Form 1095-C.

Choose Setup > Employees, click the Personal tab, click the Part II: Offer and Coverage button, and then click the Ellipsis ellipsis button button for the reporting year.

Fields & Buttons

January - December Monthly Information grid. Choose the Box 14 coverage code, enter the lowest cost monthly premium amount, and choose the Safe Harbor code (if applicable) for each month.
