Massachusetts allows forms M-941W and M-941D to be included with your MA941X file. If your filing status is Weekly (as designated on the Withholding tab of the Supplemental Information dialog), you must choose which of these forms to file.

When the Massachusetts Payroll Tax Processing window is active, choose File > Print and then mark the Create MA941X file checkbox. Click the OK button. If the current client is specified as a weekly filer in the MA Supplemental Information dialog, the Create MA941X File dialog displays.
