Effective May 3rd, the 2017 version of UltraTax CS requires multi-factor authentication (MFA) to access the electronic filing transmission feature in CS Connect.

Form 8822 - Change of Home Mailing Address

If you are a representative signing for the taxpayer, attach a copy of your power of attorney to Form 8822. For more information, see Form 8822 and Form 8822-B - Change of address FAQs.

Enter the prior name in this field if the taxpayer changed their name because of marriage, divorce, or other life events.

Note: To prevent any processing delays be sure to notify the Social Security Administration of the new name to ensure the permanent records match the name on the tax return.

If you enter code 1 or code 2 in this field, the new address information entered on Screen 8822 in the New mailing address field will proforma to Screen 1040 next year.

The application uses the old mailing address entered in Screen 1040 or, if different, the address entered in Screen 8822 to determine where to file Form 8822.
