California Franchise Tax Board requires a taxpayer to complete and submit Form 3504, Enrolled Tribal Member Certification, to support certain qualifying subtractions to income on Schedule CA(540).  If filed electronically, FTB requires Form 3504 to be attached as a PDF.  However, Form 3504 requires a handwritten signature from the taxpayer, so if you complete Form 3504 in the non-calculating folder you will need to take the following steps in order to include the PDF with the electronic record:
  1. Upon completing Form 3504 in the non-calculating folder, print a hard copy.
  2. Have taxpayer sign the hard copy.
  3. Scan the hard copy (with signature) and create a PDF. 
  4. Use Edit > Electronic Filing Attachments to attach the PDF to the California return. Choose CA Supporting Documentation – Form 3504 (Enrolled Tribal Member Certification) from the list of pre-defined Attachment Titles for 1040-CA.

For an electronically filed return, the application displays a diagnostic message under the following conditions:

  • An adjustment in the Other income statement dialog on Screen CAAdj is designated as Form 3504 exclusion but Form 3504 is not printing.
  • Form 3504 is printing but the Other income statement dialog on Screen CAAdj does not indicate which adjustment to income qualifies for the Form 3504 exclusion.
  • Form 3504 is printing but the PDF is not attached to the electronic record.
