Generating Form 4852 when filing electronically (1040)

If you have Form 4852 (a substitute for Form W-2 or Form 1099-R) you can still e-file the return if you have the employer or payer's EIN and address.

  1. Open the return that will be filed electronically.
  2. On the W2 screen in the Income folder and/or the 1099R screen in the Retirement folder:

    • Enter the EIN
    • Enter N (Non-standard) in the Standard/Non-standard W-2 field.
    • Enter the Form 4852 determination... and Form 4852 efforts to obtain... fields.
    • Enter the employer/payer address.
  3. File the return electronically.

Form 4852 is not included in the electronic file. Instead, the ERO must retain Form 4852 in the same manner as required for Form W-2 and 1099-R. Refer to IRS Publication 1345 for more information regarding IRS electronic filing requirements for Form 4852.
