Amounts entered on Screen W2 in Box 14 with Code 32 will transfer to the Wisconsin Medical Insurance Worksheet, Worksheet 2 - Others, Line 5. This worksheet can be found in Forms View under the Hlth Med folder.
Do not include insurance premiums paid by an employer unless the premiums are included as wages in Box 1 of the taxpayer's Form W-2. Premiums that are deducted pre-tax are not included in Box 1 of Form W-2.
