Per Pennsylvania DOR instructions, when the Pennsylvania return is being filed electronically, Form PA-8453 prints when a PIN is not being used as a signature. Form PA-8453 will also print if the PA return is being filed as a state-only return (without a federal return attached) and the federal return is signed using the federal self-select PIN program. Form PA-8453 also includes electronic funds withdrawal and direct deposit information.

A jurat / disclosure statement is printed when the self-select PIN program is used and the Pennsylvania return is filed with the federal return.

Form PA-8879 will print when the Practitioner PIN program is used or the ERO enters the taxpayer(s) PIN.

Notes
  • Federal self-select PINs are not considered valid signatures on amended returns.
  • These forms do not need to be sent to the PA DOR. They do, however, need to be retained by EROs and transmitters for three years after the due date of the return, or date the return was filed electronically, whichever is later.
